HOW DO I...
MAKE
AN APPOINTMENT?
A full appointment system is employed, but urgent cases will always be seen the
same day.
Please do not telephone for appointments before 8.30am, at night or at weekends.
Urgent and non-urgent appointments: (01789) 763293
HOW DO I...
REGISTER
You can register by asking at reception for a new patient form. Please note, being given a form does not automatically mean the applicant is being accepted onto the practice list.
If possible, patients can see whichever doctor they choose. However, as some of the doctors are part-time it may not be possible to see the doctor of your choice. Availability will also be affected by periods of annual leave.

When ringing for results you should be prepared to identify yourself, saying what
tests were carried out and when.
Pregnancy test results can only be given to the patient herself and not over the
telephone.
Approximate times of results returning to the doctor:

OBTAIN
A REPEAT PRESCRIPTION?
Telephone (01789) 762316 only between 10.00am - 3.00pm.
If you are taking regular medication your doctor will have told you that you can have a repeat prescription without an appointment. To do this you must be able to give the receptionist your personal identification number and the name of the medicine you require. These details are on the attached sheet of your previous prescription. 48 hours are required to process a repeat prescription so remember to order your medicine before you run out. Every six months there will be a message on your prescription asking you to see the doctor or the nurse. This is to check your health and ensure no changes are needed to your present medication.
The receptionist cannot authorise any change or renewal of your medication.
If you supply an SAE we will forward your prescription by post.
